Wisdom teeth are the last molars of each dental arch that usually erupt around late teens, that is why they are referred to as “wisdom tooth” which usually due to deficient space may get misaligned or malpositioned.

What Causes Wisdom Teeth to get Malaligned?
The wisdom tooth is the last tooth to erupt in a dental arch. There have been various theories prevalent since ages that depict impaction of wisdom tooth
- Limited jaw theory – daily jaw activities, motion of the masticatory apparatus have been declining due to changes in eating style and social background leading to reduced temporomandibular functionality. Greater wisdom tooth impaction prevalence in developed countries and in urban areas of developing countries have been reported as compared to rural areas due to traditional rural lifestyle and mastication pattern. Slow and small range of chewing food became popular in modern society, hence, limiting jaw movement during eating.
- Evolution theory – With evolution, there has been a reduction in functionality of the mastication apparatus, due to refinement in food / eating habits, which has caused a decrease in the length of the jaw, hence provided insufficient space for the inclusion of wisdom tooth in the dentition.
- Another part of evolution theory states that an increased brain size is at the expense of the jaw size. Hence, the jaw has become too small for the wisdom tooth to erupt normally.
- Growth and development theory – this theory states that impaction of wisdom tooth is mainly due to improper growth of the jaw. For example as in response to endocrine factor which regulate the growth of the tongue and mastication muscles.
How Do I Know That I Have A Need to Undergo its Removal?
A wisdom tooth demonstrates symptoms only when it becomes infected, damages other teeth or causes other dental problems, such as :
- Red / swollen / bleeding gums
- Trismus / difficulty in mouth opening
- Pain / may be radiating
- Bad breath / Any unpleasant taste in your mouth
- Pathologies (cysts, tumors) in chronic stages
If symptoms persist for a longer duration, it is best to get them removed.

- Pericoronitis – It is an inflammatory gum condition, causing severe pain , trismus and sometimes pus discharge.
- Cysts – The sac around the wisdom tooth can fill with fluid, semi fluid or gaseous content, forming a cyst that can damage the jawbone, teeth and nerves.
- Cellulitis – a bacterial infection in cheek, tongue or throat may occur.
- Sinus problems – such issue arise with maxillary wisdom tooth. They can push and rub against the sinuses putting pressure on them leading to sinus pain, pressure, headaches, and congestions.
Although it is not a common problem as sinus lining is usually distant from the tooth.
