The patient reported missing upper anterior teeth following a traumatic accident. The chief inquiry was on getting a permanent replacement for his missing tooth. The patient had visited other dentists before reporting to our clinic and was not satisfied with the treatment protocol he had advised elsewhere.

Overview of the Case :
The loss of anterior teeth has serious social consequences for the sufferer and considerably impacts appropriate social integration. Trauma, periodontal disease, dental caries, and persistent oral habits can contribute to the loss of anterior teeth. The anterior teeth can be replaced to assist in restoring anterior guidance, which is essential for the temporomandibular joint, posterior teeth, and muscles.
Detailed Description :
When the patient reported to our dental clinic in Ashok Vihar, Delhi, Dr. Rajat Sachdeva took a thorough case history of the patient. On the clinical examination, Dr. Sachdeva noted missing anterior teeth with buccal bone loss.
On careful treatment planning, our dentist suggested the patient go for implant treatment with bone grafting. The patient was explained the procedure steps and given an estimate of the treatment cost for the entire procedure before starting with his treatment.
Surgical Procedure :
Dr. Rajat Sachdeva, a highly-qualified and experienced dentist in Delhi, planned to replace the anterior teeth by placing three implants at the position of 11,21,22. The implant our dentist chose for this particular patient was Straumann SLActive because of its high-performance surface with extensive healing potential. It has excellent biocompatibility and accelerated osseointegration properties. It is also used in this type of challenging treatment protocols cases. This type of implant helps avoid bone augmentation, reduces invasiveness, and creates treatment opportunities for patients with insufficient bone.
The scans of this patient showed a very thin buccal plate which has to be taken care of while placing the implant. When the buccal plate is partially or entirely removed, the major therapeutic goal is to protect the socket until it grows for good implant stability and aesthetic results.
As there was damage on the buccal plate, our seasoned dentist Dr. Sachdeva, applied a new technique to augment the buccal plate with adequate reconstruction. The procedure for the reconstruction is the “sandwich” technique. The sandwich bone augmentation (SBA) is performed simultaneously with implant placement. It utilizes cancellous and cortical bone allografts in separate layers together with a collagen barrier membrane to simulate a healing environment similar to the composition of bone.
In this case, our dentist used PRF (platelet-rich fibrin) and Bio-Oss grafting material followed by a collagen membrane. Dr. Sachdeva sutured the surgical site to facilitate adequate bone regeneration and buccal plate reconstruction. Following this, the patient had been given a removable retainer for four months to let good wound healing and osseointegration of the implants placed.
After four months, the patient reported back for a permanent prosthesis. Adequate radiographic examination was done to see the bone growth and bonding of the implants to the bone surface.
On receiving satisfactory results treatment plan was drawn for permanent prosthesis on implants. The patient was sent back with temporaries and recalled for placement of the permanent prosthesis. The result obtained was up to the mark and the liking of our patient. Adequate occlusion was obtained, and missing teeth were successfully replaced.
